When do the first signs start?
The fertilised egg implants about 6-10 days after ovulation and the pregnancy hormone (hCG) starts to rise. Most symptoms are caused by hCG and progesterone, so they are usually noticed around the time of the missed period or a few days later.
Common early signs
- Missed period: the most reliable sign for women with regular cycles.
- Breast tenderness and fullness: often the first change noticed.
- Tiredness and sleepiness caused by progesterone.
- Nausea: usually from weeks 5-6, at any time of day despite the name "morning sickness".
- Frequent urination.
- Mild cramps and spotting: light pink-brown spotting can occur at implantation.
- Sensitivity to smells, appetite changes and mood swings.
None of these alone proves pregnancy, since similar symptoms can occur before a period, and having no symptoms does not rule it out.
When to test
- Home urine test: reliable from the first day of a missed period, best with first-morning urine. If negative and your period does not come, repeat in 2-3 days.
- Blood beta-hCG: earlier and quantitative. After IVF it is checked 9-12 days after embryo transfer.
After a positive test
Start folic acid if you are not taking it, stop smoking and alcohol and check your medicines with your physician. The first visit is usually at 6-8 weeks to confirm an intrauterine pregnancy and heartbeat; it is planned earlier after IVF or a previous ectopic pregnancy.
Seek care immediately
With a positive test, severe one-sided pain, shoulder pain, dizziness or heavy bleeding need urgent assessment for ectopic pregnancy or miscarriage.
