Intrauterine Insemination (IUI): What Is It and Who Is It For?

In short: Intrauterine insemination (IUI) places laboratory-prepared sperm into the uterus with a thin catheter at the time of ovulation. It is used for couples with open tubes, adequate ovarian reserve and mildly reduced sperm parameters. The procedure is quick, does not require anaesthesia and is usually not painful.

How is IUI done?

IUI is usually combined with low-dose medication to support ovulation, aiming for one or two follicles. Follicles are tracked by ultrasound, and a trigger injection is given when a follicle is mature.

About 36 hours later, the partner's semen sample is washed in the laboratory to select motile sperm, which are placed in the uterus with a thin catheter. The procedure takes a few minutes, followed by a short rest.

Who is it for?

  • Mild to moderately reduced sperm count or motility
  • Unexplained infertility
  • Ovulation disorders such as polycystic ovary syndrome
  • Mild endometriosis
  • Cervical factors or sexual function problems

At least one open tube and sperm parameters above a certain level are required.

IUI or IVF?

With blocked tubes, severe sperm problems, reduced ovarian reserve or advanced maternal age, IVF may be more appropriate from the start. IVF is also considered after unsuccessful IUI cycles. The decision is based on all of the couple's test results.

What are the risks?

IUI is safe. The main risk is multiple pregnancy when medication produces several follicles, so follicle growth is monitored closely and the cycle may be postponed if too many develop.

How many IUI attempts are recommended?
Usually 3-4 attempts; most pregnancies occur in the first cycles. After that, IVF is considered.

A blood test (beta-hCG) is done about 14 days after insemination.
